Claude Code was reusing a cached organisation policy after an account switch

Version 2.1.271 fixes a stale permission set staying in force until the next hourly check, after a user changes credentials mid-session. A second fix covers tool and command lists not updating when a policy loads after startup.

Anyone administering several Anthropic organisations from one machine was governed by whichever they opened first.
